Five names
come back.
Forty-five percent of patients now ask AI for a recommendation — up from six percent a year ago. Every engine returns a short list. You are on it, or you do not exist.

The part agencies miss
One question. Four engines. Four different answers.
Each engine reads a different index, so being named by one tells you nothing about the other three. An agency optimising for Google is working on a quarter of the problem.

More likely cited
The work is not on your website.
Brands are roughly six and a half times more likely to be cited through third-party sources than their own domain. For local healthcare the gap is wider still.
Engines resolve recommendations from Business Profile, Bing Places, Foursquare, Yelp and review platforms — then rank what they find by review quality and recency. A site redesign moves none of it.
Every incumbent agency sells on-site optimisation. The recommendation layer is somewhere else entirely. That gap is the whole business.
How it starts
Three steps, and the first one is free to look at.
-
Baseline
We run fifty real patient prompts across ChatGPT, Gemini, Perplexity and Copilot for your markets, and show you exactly who gets named instead of you.
-
Build
We fix the sources those engines actually read — listings, structured data, review velocity, third-party and community presence — in the order that moves your score fastest.
-
Compound
Every month the score is re-measured and reported, with the citation source behind each mention. You watch the number move and you know what moved it.
3 industries
Multi-location operators only.
Three or more locations, franchised, or platform-backed. We do not take single-location clients — the work compounds over six to twelve months and that timeline only makes sense at scale.

Before you ask
The four things people say first.
"You cannot actually measure this."
We measure it every month
Fifty prompts, four engines, three runs each — logged with the source every engine cited. The number is reproducible and you get the raw log, not just a summary.
"We already have an SEO agency."
Most of our clients do
The work barely overlaps. Theirs happens on your website. Around four fifths of ours happens off it — directories, review platforms, community and editorial. We will tell you plainly if we think you are paying twice.
"This is too new to spend on."
The shift already happened
Forty-five percent of consumers now ask AI for local recommendations, up from six percent a year earlier. The question is not whether it arrives, it is whether you are named when it does.
"Show us it works first."
That is exactly how we start
Every engagement opens with an audit. You see your score, your competitors’ scores, and the gaps — before committing to a retainer. If the number is already good, we will say so.
Comparison
What a traditional SEO retainer does not cover
Not a criticism of SEO — it still matters. These are simply different jobs, and most operators are only buying one of them.
| Traditional SEO agency | First Five | |
|---|---|---|
| Engines covered | ChatGPT, Gemini, Perplexity, Copilot | |
| Where the work happens | Almost entirely on your website | Roughly four fifths off it, where citations come from |
| What success looks like | A position on page one | Being named in a list of five |
| Primary data source | Google Search Console | Business Profile, Bing Places, Foursquare, Yelp, review platforms |
| How reviews are treated | Reputation management | A primary input to whether you get named |
| Monthly deliverable | A traffic report | A score, a per-engine breakdown, and a citation source map |
| When you are not cited | No visibility into why | The exact source that produced a competitor mention instead |
| What compounds | Rankings reset with each core update | Presence in the sources engines read |
Every row above is checkable. If your current agency covers a row we have marked, tell us and we will correct it.
This is what moving looks like.
The same market, six months on. Not a mockup of a dashboard — the actual artifact your patients see, with your name in it.

10 questions
Questions we get asked
How long before the score moves?
Directory claims and structured data can register within two to four weeks. Review velocity and third-party presence compound over three to six months. We report monthly either way, so you see progress before it is complete.
Can you guarantee we get recommended by ChatGPT?
No, and you should be sceptical of anyone who does. No AI platform publishes how it selects sources. We work from measured output and correlations documented across the industry, we report honestly, and we re-baseline the source map quarterly because these systems change fast.
How is this different from SEO?
Traditional SEO gets a page ranked. This gets a business named. The inputs barely overlap — most of the work sits in directories, review platforms, community discussion and editorial coverage rather than on your site.
Do we need to leave our current agency?
No. Most of our clients keep theirs. The work is largely complementary, and we will tell you directly if we think the two overlap enough that you are paying twice.
Why do you only work with multi-location operators?
The work compounds over six to twelve months. That timeline only makes financial sense for an operator with the budget and the patience to see it through. Taking a client we cannot serve well is worse than not taking them.
How do you measure something the AI companies do not publish?
By observation, not by insider access. We run a fixed prompt set repeatedly across four engines and record the outputs and the sources each one cites. It is empirical measurement of behaviour, not a claim about internal ranking factors.
What if our website is out of date?
It matters less than you would think. Most of what determines whether an engine names you sits outside your site. We will flag genuine website blockers — missing structured data, blocked AI crawlers — but a redesign is rarely the fastest path to a better score.
Why a six-month minimum?
Because anything shorter sets both sides up to fail. Review velocity and third-party presence do not move in ninety days, and a three-month contract encourages the kind of shortcut work that does not last.
Do you work outside the United States?
The US is our primary market today. The UK and Australia follow, and those need genuinely separate work — different vocabulary, different directories, and different advertising regulators such as AHPRA and the ASA.
What actually happens in the first thirty days?
Week one is the baseline audit. Weeks one to four are entity foundation — structured data, listing claims across every location, and confirming no AI crawler is blocked. Review velocity starts in week two. You get your first full scorecard at day thirty.
Be the practice AI gets named
Fifty prompts, four engines, your markets. You will see your score and exactly who is being named instead — before you commit to anything.
