Where does the threshold come from?

Observed behaviour across AI engines puts the practical entry point near 30 reviews at 4.3 stars or better, rising above 100 in competitive metros. It is a documented pattern rather than a published rule, so treat it as a floor to clear rather than a target to hit exactly.

Why does recency matter as much as volume?

Engines are answering "is this place currently good", not just "is this place good". Two hundred reviews with nothing in six months consistently loses to sixty updated weekly.

Is there a ceiling worth chasing?

Past roughly double the local median there are sharply diminishing returns. Getting from 20 to 60 matters enormously; getting from 300 to 340 does not.

Can we just buy reviews?

No. Every platform detects it, penalties are severe and often permanent, and for a healthcare operator the regulatory exposure is worse than the marketing loss. Build the request into discharge and follow-up instead.
